Employee Flu Shots: Thunderbird campus

All benefits-eligible ASU employees are welcome. Non-benefits-eligible employees may receive a flu shot for a nominal fee (cash or check only). HIP participants can earn 50 HIP points.

To receive a free flu shot you will need:

• a valid state-issued (Arizona) employee insurance card
• a valid ASU ID

The release is color coded. Please print the attached release in color, complete all questions and bring to the event.

There are no exceptions to these requirements. Shots will be given on a first-come, first-served basis for as long as the supply lasts.

Non-benefits-eligible employees can also receive flu shots, but the shots will cost $20, payable in cash or by check only. Employees' family members, retirees, student employees, graduate assistants and other students are not eligible to receive a shot through this program.
