MOM: Mobile On-site Mammography – Polytechnic

Mobile On-Site Mammography will be available to benefits-eligible state employees on April 20 at the Polytechnic campus. The free event is done by SimonMed.

Women under age 35 will need a doctor’s referral. Baseline mammograms start at age 35 to 39. Most insurance plans cover mammograms every year after age 40.

Please bring the following items to your appointment:

• Current health insurance card.
• Locations of your previous mammograms.
• Your doctor’s name and address.
