Mobile On-site Mammography by SimonMed will be at the Downtown Phoenix, Tempe, Polytechnic and West campus. MOM’s professional staff uses the latest 3-D digital technology. Appointments required, schedule your own appointment online or by calling 480-967-3767/ 1-800-285-0272.

What to bring:

·        Doctor’s name and address.

·        Insurance card.

·        Previous mammogram location.

·        Please wear a mask during the appointment.

Recommended baseline mammograms age 35-40. Recommended annual age 40 & over if you’re at average risk.  Most insurance plans cover them yearly after age 40.
